In-home Healthcare Services
Companion Care
Companion Care services are available to clients of all ages who are recovering from hospitalization or illness, those who have chronic medical conditions, or who are frail, elderly, disabled, or simply require additional assistance with activities of daily living (ADL’s) in order to safely remain at home.
Foundation Services
Representative Payee
The Representative Payee program provides financial management assistance to at-risk clients, including elderly, disabled, court protected minors and adults, mental health clients, recovering substance abusers and any other person requiring assistance with the administration of their finances.
Caregiver Training & Assistance
The Caregiver Program assists community and family caregivers by providing hands-on skills training and education regarding proper care and handling of individuals in a home setting. Other assistance includes support groups, indentifying and providing access to available community resources, help with questions about specific medical conditions, and workshops focusing on various topics of importance to caregivers.
