Companion
Care
Companion Care services are available to clients of all ages
who are recovering from hospitalization or illness, those
who have chronic medical conditions, or who are frail, elderly,
disabled, or simply require additional assistance with Activities
of Daily Living (ADL's) in order to safely remain at home.
Home Care
Home Health Care services are available to patients of all
ages with acute illness, long term health conditions, temporary
or permanent disability, terminal illness, or in lieu of hospitalization
as determined by the patient's physician, in order to remain
at home.
Linkages Case
Management
The Linkages Program provides no cost community assistance
and case management services for elderly and disabled adults.
Specific services include, but are not limited to, information
and resource assistance, links to existing community services,
medical and financial advocacy and professional case management.
Homemaker Program
The Homemaker Program provides basic home and personal care
services to senior adults in the outlying areas of Kern County.
Participants pay a sliding fee portion of costs based upon
income, with the balance sponsored by local donations and
county grants.
Payee Program
The Representative Payee Program provides financial management
assistance to at-risk clients, including elderly, disabled,
court protected minors and adults, mental health clients,
recovering substance abusers and any other person requiring
assitance with the administration of their finances.
Caregiver Training
and Assistance Program
The Caregiver Program assists community and family caregivers
by providing hands-on skills training and education regarding
proper care and handling of individuals in a home setting.
Other assistance includes support groups, indentifying and
providing access to available community resources, help with
questions about specific medical conditions, and workshops
focusing on various topics of importance to caregivers.
